Make Program and Form names | Grantmaking and Foundations Ideas

Make Program and Form names available as fields in a Dashboard part and on the Request screen.

We can only see the Program and Form name by opening the application from the Documents tab of a Request. I looked for these fields in the Request table and don't see them. I would like to display both fields on both the Request record and in a Dashboard part. Please make them available in both places.
