Update titles for contacts when an online application is submitted

The title for a contact is not a field which is updated. Instead, a new affiliation record is created for the request, as the contact is created as related (affiliated) with the request. This creates an unnecessary amount of duplicate affiliates while not updating one filed for a contact.
