Don't auto-populate request, requirement fields & uploads with old data from previous forms/years submitted

Users in our program can apply multiple times to our program, year after year. Testing is showing that when an applicant completes a 2nd+ request record, the Uploads carry over from the last time they applied. We are looking for new data each time a user submits a request (a new tax return each year; a new transcript each year; etc.)- please disable this "auto-filling" feature, or at least make it optional.

The same problem is evident in the Requirement forms. Our program uses Requirements to make payments contingent. Our users can have up to 4 payments per year. Current testing shows that if a requirement form is re-used, the last data provided by the user pre-populates in the subsequent requirement to release the subsequent payment.

I'm asking our users for new information with each request and requirement - however I'm going to receive old information, because the form is pre-filled from the last time a user completed the form. Now the work is on our staff, and the user, to first delete the old data, then enter the new data, in the form. The forms should arrive as a blank slate, so that the user can fill out the form/upload the appropriate document only once. This was not an issue in IGAM, so I don't understand why these fields are created this way in the new software.
